Health Care Aide
This position involves:
-Review with Nurse Manager the initial assessment and proposed care plan for the client.
-Provide the desired care though the duties listed below.
-Report significant changes in the client's health status to the Realcare Office/Nurse Manager immediately.
-Document care given in the Realcare progress notes.
Private Care Duties
-Shopping duties.
-Transport to and from appointments.
-Assisting with walking.
-Monitoring vital signs.
-Assisting with range of motion exercises, one person transfer/lift.
-Prompting with medication taking.
-Meal preparation and feeding assistance.
-Providing client hygiene (i.e. Washing, bathing/showering, mouth care, toileting, peri-care).
-Providing companionship through conversation and/or activities.
In a facility we are there to assist the staff in providing quality client care. Our staff takes their directions from the facility staff. If there is a problem with the instructions you are given, you can take your concerns to the charge nurse. If you still have a problem, you can call the office any time. Never leave a facility during your shift without first contacting the office.
Institutional Care Duties
Providing quality care in facilities within the framework of his/her educational competencies and within the guidelines of the specific facilities which can include:
-Maintain and promote client hygiene
-Maintain and promote client exercise and rest
-Maintain and promote client nutrition
-Maintain and promote client elimination
-Maintain and promote client safety
-Maintains the client environment in a safe clean and pleasant state.
-Participates in unit response to emergency situations
-Contributes to team goals
-Performs other related duties as required

Private Care Duties
Assessing:
-Obtains or reviews with the Nurse Manager the initial assessment of the client.
-Makes direct contact with the attending Physician as required.
Planning:
-Develops and revises with the Nurse Manager an individualized care plan.
Implementing:
-Provides specific nursing care to achieve the planned objectives/outcomes.
-Encourages client to be an active participant in his/her care by helping him/her to fully understand the care path
-Reviews implementation of the care path with other health team members.
Evaluating:
-Monitors client's progress as related to the objectives out lined in the care plan.
-Revises care plan when necessary in consultation with the Nurse Manager.
Institutional Care Duties
-Providing quality care in institutions within the framework of his/her educational competencies and within the guidelines of the specific institution:
-Client hygiene
-Monitoring vital signs
-Administer tube feeding
-Colostomy care
-ROM (range of motion) exercises
-Urine testing
-Dressing changes
-Monitor irrigate/remove nasogastric tube
-Administer tube feeding
-Set-up, administer medications— NO I.V. meds Administer IM medications upon proof of certification
-Insert (upon proof of certification), irrigate and remove indwelling Foley catheters
-Decubitus ulcer care
-Apply binders, bandages
-I.V. monitoring, change bags —info added medication, change tube, removal
-Record all care given including clients response/reactions
-Encourage client compliance to care path and provide emotional support when needed

As we set out to say farewell to Mom at her funeral today our overwhelming feeling, emotion is one of gratitude – not least for the angels from Realcare Inc. who looked after our mother as if she was their own.
